Student-athletes, have you already done your sports physique?


Administrators, coaches, parents, athletes and fans are all hoping that fall sports start on time this year. FHN hopes so too!

We’re proud to offer our $ 20 School Sport Workouts this summer for Northwestern Illinois student-athletes. As in the past, each exam is $ 20 (payable by cash or check only) and we will be returning the fees to the school later this summer.

COVID-19 has made some necessary adjustments, such as a stricter cleaning protocol and mandatory masks for everyone entering the school building or clinic. Students will stay in a room for the exam, and the approximate exam duration is 15-20 minutes. We ask that only one parent or guardian accompany each student-athlete; please do not bring siblings or people who will not receive a medical examination.

All forms (an FHN Consent Form and the IHSA Examination Form) must be completed and signed before taking a sports physical exam. You can get these forms from most schools in the area or download them from The physical files of all the students will be given to the school at the end of each evening. Athletes can take a photo of the completed form with their phone for their records.

1 p.m. to 5 p.m. Wednesday, July 7 at Pecatonica High School, 1300 Main Street. Call the clinic at 815-239-1400 for an appointment.

5:30 p.m. – 7 p.m. Wednesday July 7 at FHN Family Healthcare Center – Highland View Drive, 3001 Highland View Drive, Freeport. No appointment is necessary.


2 p.m. to 5 p.m. Thursday, July 15 at the Jo Daviess Family Healthcare Center, 600 N. Rush St., Stockton. Call the clinic at 815-947-3211 for an appointment.


3 pm to 6 pm Thursday, July 15 at FHN Family Healthcare Center – Lena, 160 W. Main St. Call the clinic at 815-369-3300 for an appointment.


1 p.m. to 3 p.m. Thursday, July 22 at Warren High School, 311 Water Street in Warren. Call the clinic at 815-745-2644 for an appointment.


3 pm to 5 pm Tuesday July 27 at Orangeville High School, 201 S. Orange St. No appointment required.

Free port

Freeport’s athletic physical exams will be performed at the high school (701 W. Moseley St.) again this year to ensure social distancing in the gymnasium while providing a safe environment for participants due to concerns about COVID-19. Please arrive during the designated time for your student’s sport (below) to ensure that the ability to participate meets appropriate safety standards. Please park in the west parking lot as students will enter through the Empire Street entrance and exit onto the west lot.

Wednesday July 21

5 pm-5:30pm – Football, cross-country and golf

5.30 p.m.-6 p.m. – Soccer and dance

6 pm-6.30pm – Volleyball, tennis and swimming

6:30 p.m. to 7 p.m. – NJROTC and softball

Tuesday August 10

5 pm-5:30pm – Ghosts and wrestling

5.30 p.m.-6 p.m. – Basketball, bowling and cheers

6 pm-6.30pm – Baseball, athletics and softball

6.30 p.m.-7 p.m. – All sport
